The very high pressure goes through a very small area of the base of the stiletto heel, not the foot. The heel widens out to support the foot. As for the maximum height of heel, it depends entirely upon the wearers ability to balance, and I believe that the five inches you quoted is guesswork. (not necessarily by you. Sorry.
